What Is Hypothyroidism?

The thyroid gland is located in front of the neck and is small, like a butterfly. It makes hormones, primarily Thyroxine (T4) and Triiodothyronine (T3), that play different key roles in the body and its metabolism. Hypothyroidism occurs when the thyroid gland makes inadequate amounts of thyroid hormones. This can cause several body functions to slow down.

Hypothyroidism may happen at any age and can be progressive. It is sometimes diagnosed in people who are having regular blood tests, before symptoms become noticeable. For other people, symptoms might prompt them to request medical advice. Once the cause has been determined, treatment may be started, and then the condition can generally be managed to a satisfactory level.

These are not the only symptoms of hypothyroidism. Some people may experience just one or two changes. If symptoms persist, you may want to have your thyroid levels tested to see if an underactive thyroid is causing the symptoms.

How Is Hypothyroidism Diagnosed?

A blood test is the most important means for the diagnosis and monitoring of hypothyroidism.

TSI & Initial Screening
Thyroid-stimulating antibody (TSI) is often ordered as a first test of thyroid function.

Free T4 & T3 Evaluation
Doctors might also recommend free T4 and other thyroid tests. T3 can be ordered in some cases but is not always ordered when hypothyroidism is simple.

Thyroid Antibody Testing
When an autoimmune cause (like Hashimoto's disease) is suspected, thyroid antibody testing may also be ordered.

Clinical Correlation
Symptoms, medical history, and examination findings should always be taken in conjunction with results. An abnormal value may not represent the whole picture.
Nuclear Imaging

Thyroid Scan and Hypothyroidism

A Thyroid Scan is primarily used to assess the structure and functional activity of the thyroid and is more commonly useful in selected cases of hyperthyroidism, thyroid nodules, and other specific thyroid conditions. It is not routinely required for every patient with hypothyroidism.

However, there are situations where a doctor may recommend additional thyroid imaging depending on the patient's symptoms, examination findings, history, or an unusual thyroid function pattern. At Molecular Diagnostics and Therapy, the Nuclear Medicine team can assess whether a thyroid scan or another investigation is appropriate based on the individual case.

Treatment for Hypothyroidism

Levothyroxine Therapy
The treatment of hypothyroidism depends on its cause and severity. Most people with hypothyroidism are treated with thyroid medication. Levothyroxine is used to treat hypothyroidism (inadequate thyroid hormone production). It isn't always the same dose for everybody. It may be influenced by age, body weight, levels of hypothyroidism, other illnesses, or thyroid surgery or radioactive iodine medication.
Routine Blood Tests & Dosage Adjustments
Normally, blood tests are performed during treatment to see if the dosage is correct. Dosage adjustments may be made according to TSH and/or other appropriate results.
Physician Guidance & Patient Compliance
Unless directed otherwise by the physician, each patient is advised to take his/her thyroid medication as prescribed and not to alter the dose or stop it without first consulting the doctor.
Oncology Follow-Up Care

Thyroid Cancer Follow-Up Care

If a person's thyroid is surgically removed, they do not make sufficient thyroid hormones on their own. Replacement treatment with thyroid hormones is thus usually necessary. There may also be a decrease in thyroid function in patients who have used radioactive iodine for treatment in the past. In these cases, thyroid hormone replacement might be needed. Anyone who has had thyroid cancer treatment may have long-term follow-up that includes management of thyroid hormone. Individual patients with cancer may also have a different target TSH based on their cancer type and treatment, however. That is why it is important to follow up on a regular basis with the treating physician.

Hypothyroidism and thyroid cancer are different conditions, but they can sometimes be connected through treatment. A patient who has undergone surgery for thyroid cancer or received radioactive iodine may need lifelong thyroid hormone replacement. Such patients may also require periodic monitoring with blood tests, ultrasound, thyroglobulin testing, or Whole Body Iodine Scanning when clinically indicated.
